Volunteer Conflict of Interest Policy and Statement of Disclosure Form

The key to recognizing and dealing with Conflicts of Interest is disclosure. Full and accurate disclosure regarding actual, potential, or perceived Conflicts of Interest is in the best interest of both OrHIMA members and the organization. All individuals engaged in volunteer activities for OrHIMA are required to complete this form on an annual basis.
